Job summary

Care Comfort Hospice is seeking a Visit Nurse to deliver compassionate hospice care to patients and families in the community. You will work with the Nurse Case Manager, DON, and Interdisciplinary Team to implement the patient’s care plan.

Orientation will be provided. The role requires on-call responsibilities and adherence to hospice ethics and CQI activities, with a flexible schedule across shifts.

Qualifications

• Active PA RN license in good standing.
• 1–2 years’ acute care nursing or 1 year in hospice/home health preferred.
• Desire to learn and commitment to hospice philosophy and comfort care.

Responsibilities

• Provide hands-on clinical nursing, interacting with patients, family, physicians, and the care team.
• Assess and coordinate a pain/symptom management plan with patient, family, and Interdisciplinary Team.
• Assist in facilitating end-of-life comfort and dignity for patients and families.
• Educate patient and family on disease process and expectations.
• Coordinate referrals for home health aides, chaplains, social workers, and other services.
• Update the Plan of Care (POC) with the Interdisciplinary Team as patient needs evolve.
• Ensure services align with the patient’s POC and active participation of family and care team.
