Qualifications
- Nursing degree or diploma required (Diploma/Associate/Bachelor)
- Current state RN license
- CPR certification current
- Compassionate, friendly demeanor
- Excellent communication skills
- Valid driver’s license with reliable transportation
- Hospice experience required
Responsibilities
- Visit patients in their homes for one-on-one visits
- Perform initial and ongoing assessments of physical, psychosocial, and environmental needs
- Assess patient conditions, chart observations, and monitor vital signs
- Administer medications as prescribed
- Educate patients and families on home health care strategies and devices
- Communicate effectively with patients, families, team members, and other health professionals
