Job summary

VITAS Healthcare is seeking an experienced home care registered nurse (RN) to ensure hospice patients are comfortable and to build compassionate connections with families in Illinois. You will work primarily in patients' homes, visiting multiple sites and collaborating with an interdisciplinary team.

You will provide end-of-life care with autonomy at the bedside, educate families, and explain the palliative nature of hospice.

Qualifications

• Currently licensed to practice nursing in the state where the VITAS program is located.
• A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Provide care to hospice patients at home ensuring comfort and meaningful connections.
• Collaborate with interdisciplinary team during weekly office visits.
• Make bedside care decisions to extend comfort and dignity.
