Job summary
Interim HealthCare Hospice in Wisconsin is seeking a compassionate Hospice Registered Nurse (RN) to cover Hudson, WI and surrounding areas. This role provides home-based care to patients with six months or less to live, focusing on quality of life.
You will work with an interdisciplinary team to carry out the plan of care, perform assessments, monitor vitals, administer prescribed medications, and educate families on prognosis and hospice goals.
Qualifications
- Graduate of an accredited nursing program and active RN license in Wisconsin.
- Minimum of 1 year of nursing experience, ideally in hospice or home healthcare.
- CPR certification and knowledge of home health regulations.
Responsibilities
- Provide home-based care to patients with six months or less to live, focused on quality of life.
- Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
- Educate family on prognosis, changes in care plan and hospice goals.
- Collaborate with an interdisciplinary team to carry out the plan of care.
