Qualifications

• 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• Comfortable providing care in diverse settings, including private homes and facilities.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Be the primary point of contact for patients and families.
• Deliver and document skilled nursing care.
• Perform initial and ongoing assessments for patients.
• Collaborate with an interdisciplinary team for personalized care.
