Job summary

Liberty Hospice in Smithfield, NC seeks an experienced Registered Nurse to provide compassionate home health and hospice care. You will apply nursing theory, document care per reimbursement guidelines, and coordinate physical care with families and the Hospice team.

The ideal candidate holds an active RN license in North Carolina, has 2+ years of clinical nursing experience (preferred hospice experience), CPR, and is willing to travel and cover weekend/evening/holiday call as needed.

Qualifications

• Current RN licensure in state of practice.
• Two years of clinical nursing experience with at least one year in hospice preferred.
• CPR certification.
• 12 hours of continuing education annually.
• Willingness to travel and weekend/evening/holiday call as needed.

Responsibilities

• Apply nursing theories and concepts to patient care.
• Care and documentation of care meeting reimbursement guidelines.
• Coordinate physical care with family, patient, physician and Hospice team.
• Communicate with patients/families at all levels and with agency personnel.
• Attend IDT meetings and quality reviews as scheduled.
