Job summary

Enhabit Home Health & Hospice is seeking a dedicated Registered Nurse to join our team in a home-based care setting. You will coordinate care, assess patient and family needs, and implement the interdisciplinary care plan in line with physician orders.

The ideal candidate is a graduate of an approved nursing program, licensed in the state, with at least one year of RN experience and comfort using mobile devices for documentation and team communication.

Qualifications

• Graduate of an approved school of nursing.
• Licensed in the state where practicing.
• Minimum of one year experience as an RN.
• Basic technology skills, including mobile device use.

Responsibilities

• Coordinate care and assess each patient’s and family’s needs.
• Implement the interdisciplinary care plan per physician orders.
• Ensure delivery of care complies with federal and state regulations.
• Document care and communicate effectively with the care team.
