Phoenix Home Care and Hospice is seeking a Nurse Case Manager to deliver compassionate hospice nursing services in the Kansas City area. You will collaborate with an interdisciplinary team to create patient- and family-centered plans of care.

Ideal candidates will hold an active Kansas or Compact RN license, hospice experience preferred (training provided), and a valid driver’s license with auto insurance. Sign-on bonus available and comprehensive benefits.
