Hospice of the Piedmont in Virginia is seeking a PRN RN Case Manager to provide compassionate, patient-centered care that honors dignity and comfort at life's meaningful moments. You will perform nursing evaluations for new admissions and collaborate with the physician and health team.

This role requires home visits, EMR documentation, driving to patients’ homes, and flexibility to work beyond scheduled hours to ensure quality patient care. Competitive benefits accompany the position.

Formación

• Graduation from an accredited nursing school and an active Virginia RN license.
• Two years of relevant nursing experience preferred.
• Driver’s license, personal automobile, and current liability insurance.

Responsabilidades

• Perform nursing evaluation visit for new admissions.
• Regularly re-evaluate patient needs.
• Coordinate and manage services with health team members.
• Complete bedside documentation in EMR.
• Consult with the patient’s physician.
