Job summary

Best Hospice Care of Texas is seeking a compassionate Part-Time RN Case Manager to join our growing Southeast Texas team. You’ll provide skilled nursing care, coordinate plans of care, and support patients and families in home settings with empathy and professionalism.

Ideal candidates hold a Texas RN license, are organized, independent, and able to travel within the service area. Hospice/home health experience is preferred but not required as we train and integrate you with a collaborative

Qualifications

• RN license in Texas.
• Compassionate, dependable and comfortable working independently.
• Strong communication and teamwork skills.

Responsibilities

• Providing skilled nursing care to hospice patients in their homes or care settings.
• Completing patient assessments and developing individualized plans of care.
• Managing and coordinating care for your assigned patients.
• Monitoring changes in condition and communicating updates with the hospice team and physician.
• Providing education, guidance, and emotional support to patients and their families.
• Completing required clinical documentation accurately and timely.
• Participating in interdisciplinary team collaboration and care planning.
• Communicating with CNAs, social workers, chaplains, providers, and other members of the care team.
• Helping ensure patients and families feel supported, informed, and cared for throughout their hospice experience.
