Job summary

Qualifications

• Minimum of one year nursing experience in medical/surgical, oncology, geriatric, or hospice/home care.
• BSN degree preferred; CHPN or OCN preferred.

Responsibilities

• Perform patient assessments for comfort and symptom relief.
• Develop and implement an individualized plan of care for each patient.
• Educate patients and families as needed about care.
• Obtain physician orders to support symptom management.
• Coordinate home visit scheduling and mile/time management.
• Refer to IDT members and collaborate with social workers.
