Job summary

GBMC HealthCare seeks a dedicated nurse to provide compassionate care for patients with life-limiting illnesses and support for their families. The role collaborates with physicians and an interdisciplinary team to develop patient-centered care plans and educate families on care activities and infection control.

Responsibilities include monitoring patient progress, documenting care, and participating in quality improvement, while supporting families through end-of-life care and bereavement

Qualifications

• RN license active in Maryland.
• ADN and/or BSN from an accredited institution.
• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledgeable of universal precautions for infection control and employs safe practices for self protection.
• Strong time management skills.
• Excellent interpersonal and communication skills.
• Ability to be patient and family focused.
• Skill in EMR and ability to learn.

Responsibilities

• Provides quality nursing care to patients with life-limiting illness to maintain highest level of care in conjunction with the primary physician.
• Participates as a member of the interdisciplinary team to develop and implement a patient directed individualized plan of care.
• Evaluates patient/family response to intervention and modifies care plan appropriately.
• Provides patient and family education to achieve skills necessary to maintain quality care and independence.
• Documents on appropriate chart and forms in a timely manner.
• Participates in Quality Improvement Program to improve patient care.
• Supervises Patient Care Partners, Unit Receptionists and volunteers.
• Completes daily acuity assessment for assigned patient.
• Attends to patient and family at time of patient’s death.
