Job summary

The Care Team Home Health & Hospice in West Branch, Michigan is seeking a Part-Time Music Therapist to provide comfort, connection, and emotional support to hospice patients and families in home and facility settings.

You will conduct music therapy assessments, develop individualized plans, collaborate with the interdisciplinary team, and document outcomes in HCHB EMR. MI license/registration and CPR are required, with HIPAA confidentiality a must.

Qualifications

• Licensed or registered in the State of MI.
• Must pass a criminal background check & MVR check.
• Health statements acknowledging ability to perform duties.
• Valid state driver’s license.
• Proof of current automobile insurance.
• Current CPR card; online CPR acceptable; hands-on CPR preferred.
• TB testing per agency policy within 12 months of hire.

Responsibilities

• Conduct music therapy assessments and develop individualized plans.
• Provide therapeutic music interventions for patients and families.
• Collaborate with interdisciplinary team members.
• Support emotional expression, relaxation, and symptom relief.
• Document services and patient responses accurately and timely in the EMR HCHB.
