Job summary

Anchor Health in San Joaquin County seeks an experienced Hospice Medical Social Worker to support patients and families within the hospice interdisciplinary team. The MSW conducts ongoing psychosocial assessments, develops care plans, and coordinates access to community resources to meet patient needs.

Travel throughout the assigned geography may be required. A CSWE-accredited MSW (or BSW) with hospice knowledge, CPR certification, and a valid driver’s license are essential.

Qualifications

• Must have a CSWE-accredited MSW or CSWE-accredited BSW.
• Experience in hospice and providing psychosocial support.
• CPR certification and valid driver’s license required.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the team.
• Coordinate care plans and provide crisis intervention when needed.
• Educate on advance directives and connect families with resources.
• Maintain clinical records and participate in interdisciplinary group meetings.
• Support funeral planning and legal/financial arrangements as needed.
