Good Samaritan Hospice, Inc. seeks an LPN to provide compassionate, direct patient care in homes and communities under RN supervision.

You will document in EMR, manage symptoms, educate families, and collaborate with an interdisciplinary team to ensure high-quality hospice care. The role is full-time with a Mon-Fri schedule and rotating weekend/holiday on-call, requires VA LPN license and 3 years of nursing experience; starting pay is $28/hour, with comprehensive benefits and growth

Competenze

• Graduate of an accredited Practical Nursing program.
• Current Virginia Licensed Practical Nurse (LPN) license.
• Minimum of three (3) years of nursing experience; hospice/home health/community health/medical-surgical preferred.
• Experience with electronic medical records (EMR).
• Current driver's license and reliable transportation.
• Current CPR certification or ability to obtain per organizational requirements.

Mansioni

• Provide compassionate, direct patient care under RN supervision per plan of care.
• Deliver treatments, monitor conditions, and report changes to the RN Care Manager.
• Assist with pain and symptom management to promote comfort and quality of life.
• Educate patients and family caregivers about diagnoses, medications, and plan of care.
• Document all patient care, assessments, and interventions in EMR.
• Collaborate with physicians and interdisciplinary team for hospice care.
• Safely prepare/use medical equipment and follow infection control standards.
• Participate in team meetings, continuing education, and quality initiatives.
• Provide compassionate, respectful, patient-centered care in every interaction.
• Participate in nursing holiday rotation.
