Job summary

Visiting Nurse Association of SEMO is seeking a licensed practical nurse (LPN) to provide skilled hospice and home health care. The role involves delivering nursing care in patient homes or care facilities in accordance with physician orders and CMS regulations, with coordination with the IDG and R.N.

staff. The LPN will perform ongoing patient assessments, medication reconciliation, and documentation in the EMR, while participating in IDG meetings and maintaining the care plan.

Qualifications

  • LPN license in good standing.
  • Experience in hospice or home health preferred.
  • Knowledge of CMS regulations and standards for hospice care.

Responsibilities

  • Provide skilled nursing care to hospice patients in accordance with agency policies, physician orders, and CMS regulations.
  • Make pertinent nursing observations and communicate with R.N. Staff Nurse, TCM, Hospice physician and/or IDG team; document promptly.
  • Attend IDG meetings and document IDG notes.
  • Take on-call rotation as scheduled.
  • Maintain current clinical notes in EMR and update care plan accordingly.
  • Inform doctor of changes in patient condition and document in narrative notes.
  • Review medication profiles and reconcile medications weekly.
  • Assist R.N. in performing specialized duties.
  • Report identified needs or problems to the R.N. in a timely manner.
  • Follow the plan of care initiated by R.N./physician and adhere to state guidelines for LPN in homecare/hospice.
  • Provide accurate information to patients/families as outlined in the nursing care plan.
