Hospice Buffalo is seeking a compassionate Home Health Aide to support patients in Cheektowaga, NY. The role focuses on personal care, daily living activities, and nutritional support in a home health setting.

The position offers a part-time schedule with two shifts per week, including every other weekend and holiday rotations. A NYS-accepted HHA certificate is required, with a preference for those holding hospice and palliative care training.

Qualifications

• One year of clinical experience is preferred.
• NY State approved Home Health Care training with HHA certificate is required.
• High school diploma or GED preferred.
• Certification in Hospice and Palliative Care encouraged.

Responsibilities

• Assists with hygiene, grooming, bathing, nail and skin care, and personal care.
• Assists with activities of daily living and instrumental activities of daily living.
• Plans meals to meet nutritional needs and assists with feeding when necessary.
• Maintains confidentiality and follows HIPAA policies.
• Follows Standard Precautions and infection control procedures.
• Documents care provided and communicates with Interdisciplinary Team.
