Job summary

Superior Hospice is seeking a dedicated Hospice CNA to join the 21-County Twin Cities service area care team. You will assist patients with bathing, dressing, grooming, toileting and mobility, monitor comfort, and provide companionship and support to patients and families.

You’ll follow individualized care plans, work closely with nurses and the interdisciplinary team, and help create a safe, clean environment.

Qualifications

• Hospice experience preferred, with home care, assisted living, long-term care, hospital, or oncology experience also considered.
• Compassionate, observant, and dependable.
• Excellent communication and collaboration skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assist patients with bathing, dressing, grooming, toileting, and mobility.
• Monitor patient comfort and report changes in condition to the nursing team.
• Provide companionship and emotional support to patients and families.
• Help create a safe, clean, and comfortable environment.
• Follow individualized care plans while working closely with nurses and the interdisciplinary team.
• Deliver care that promotes dignity, comfort, and quality of life.
