Job summary

Hospice of the West is seeking a Per Diem Hospice Aide (CNA) to join our compassionate team in Yavapai County. You will provide hands-on care, supporting patients’ physical, emotional, and personal needs with respect and dignity, under the direction of our nursing staff.

As a CNA, you’ll follow individualized care plans, assist with daily living activities, document accurately, and contribute to a comforting experience for patients and their families during this challenging time.

Qualifications

• High School Diploma or GED required.
• Unencumbered CNA certificate in the state.
• Minimum one year CNA experience in acute or long-term care; hospice a plus.
• CPR certification current.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personalized care following the care plan to ensure comfort and well-being.
• Assist with personal care tasks, including bathing, hygiene, and grooming.
• Deliver exceptional customer service and patient satisfaction.
• Support families by sharing knowledge and guidance during illness.
• Maintain accurate documentation in home and agency records.
