Job summary

New-Age-Hospice is seeking a Certified Nursing Assistant (CNA) or Hospice Aide to provide in-home patient care across League City, Dickinson, Texas City, La Porte, and South Houston. You will support patients in their homes under RN supervision and follow the Aide Plan of Care.

The ideal candidate has a high school diploma or equivalent, CNA/HHA training, and reliable transportation. Prior hospice or home health experience is preferred, with strong communication and infection control skills.

Qualifications

• High School Diploma preferred or equivalent with approved training or competency evaluation.
• 1+ year experience as Hospice Aide or Nursing Assistant preferred.
• Must read, write, and verbally report clinical information clearly to patients, families, and staff.
• Reliable transportation and valid driver’s license required.

Responsibilities

• Promotes the philosophy and policies of the Hospice.
• Provides direct patient care under RN supervision and Aide Plan of Care.
• Assists with safe transfers and ambulation per Hospice policy.
• Reports changes and documents care rendered to ensure continuity of care.
• Maintains infection control and a clean, safe environment.
• Communicates respectfully with patients, families, and staff.
