Gentiva Hospice is seeking a dependable Hospice Aide/CNA to provide compassionate, hands-on care to patients wherever they call home — private residences, assisted living facilities, or inpatient units.

You will perform direct personal care, support daily activities, and offer emotional support to patients and families while maintaining thorough documentation and staying aligned with care plans.

Qualifikationen

• Active CNA license in the state of employment is required.
• Current CPR certification is required.
• Driver's license and a reliable vehicle with insurance are required.
• Six months of direct patient care experience is required.
• Experience working in home settings, hospice facilities, or long-term care environments is preferred.

Aufgaben

• Provide direct personal care to terminally ill patients.
• Assist with personal hygiene, grooming, and toileting.
• Support safe mobility with walkers and wheelchairs.
• Help with meal preparation, feeding, and light housekeeping.
• Observe and report changes in patient condition.
• Offer emotional and psychological support to patients and families.
• Maintain accurate documentation of visits.
• Attend team meetings and participate in quality improvement.
