Job summary

Suncrest Hospice is seeking a Certified Nursing Assistant/Home Health Aide (CNA/HHA) to provide resident care under the direction of a Case Manager/DON. You will work with nurses, social workers, chaplains, volunteers, and the patient care team to ensure high-quality care in a CHAP-certified hospice.

The role emphasizes teamwork, driving to patients’ homes, and adherence to nursing policies while advancing opportunities for certifications and growth within a rapidly expanding national company.

Qualifications

• Strong commitment to improving quality of life while partnering with the community in patient care.
• Ability to work well within a team with nurses, social workers, chaplains, volunteers, and aides.
• Willingness to pursue higher certifications and education.
• Able to work 1-1 with patients while receiving support from the care team.
• Must have reliable transportation, valid driver’s license, and auto insurance.
• Certified Nursing Assistant (CNA) certification in the state(s) of practice.
• Home Health Aide (HHA) certification in the state(s) of practice.

Responsibilities

• Provide resident care in accordance with assessments and care plans under supervision of Case Manager/DON.
• Coordinate care with licensed staff and follow nursing policies, procedures, and safety guidelines.
• Support a team-oriented approach to patient care and community partnerships.
