Job summary

Trinity Health Group is hiring CNAs/HHA for home health services, providing compassionate, patient-centered care in clients' homes in Texas. Responsibilities include following care plans, assisting with hygiene and mobility, and documenting observations for the supervising nurse.

Applicants should have TX nurse aide training/registry, be CPR certified, possess a high school diploma or GED, and be able to read, write, and communicate in English.

Qualifications

• Must read, write and communicate effectively in English.
• Reliable means of transportation to client visits.
• Must clear a criminal background check and CPR certification.

Responsibilities

• Follow the POC for each client and document.
• Help clients maintain good personal hygiene.
• Assist in maintaining a healthful, safe environment.
• Assist clients with ambulation as per the POC.
• Assist with treatments as ordered by physician and supervised by nurse.
• Take and chart vital signs under supervision.
