Job summary

Moments Hospice in Wisconsin is seeking a compassionate CNA to provide personal care and support under an RN Case Manager. You'll perform daily living activities, review care plans, and help with transfers while traveling on the road.

The role includes a competitive hourly wage of $18–$23, benefits such as health insurance, a company car with gas and insurance coverage, cell phone, PTO, and 401(k). A CNA license and 12 months caregiving experience are required.

Qualifications

• CNA license required.
• 12 months caregiving experience in Home Care/Assisted Living/Memory Care/Hospice settings required.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication and active listening skills.
• Ability to work in an electronic medical record system.
• Valid driver’s license and auto insurance required.
• Ability to lift 50 lbs and understand body mechanics.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
