Job summary
Moments Hospice in Wisconsin is seeking a compassionate Certified Nursing Assistant (CNA) to support patients' daily living activities and dignity during their final days. You will perform personal care, review care plans, assist with transfers, and work under the supervision of an RN Case Manager, using an electronic medical record system.
This role includes on-road duties with a competitive salary of $18–$23 per hour, on-call support, and a benefits package featuring a company car, health
Qualifications
- CNA license required.
- 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
- Current CPR certification preferred.
- Excellent customer service and strong communication skills.
- Problem-solving, time management, and ability to work in an EMR system.
- Valid driver's license and auto insurance required.
Responsibilities
- Perform personal care activities related to daily living.
- Review patient care plans and assist with lifting and transferring patients.
- Assist with range of motion, positioning, exercise, and ambulation.
- Assist with dining and nutrition and maintain EMR records.
- Work under the direction of a Case Manager RN.
