Job summary

Join the Cleveland Clinic team as a Hospice Aide, PRN (STNA required) in Independence. Serve patients at home, nursing facilities, and other settings under the Hospice program, providing compassionate end-of-life care and dignity.

You will work 8-hour shifts on an as-needed basis with a minimum 48 hours in a 6-week period, and collaborate with RN/PT/OT/ST staff to support patient comfort and safety. You will assist with daily living activities, document conditions and interventions, drive to

Qualifications

High School Diploma or GED required.
State Tested Nurse Aide (STNA) Certification required.
One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting.
Valid Driver’s License in Ohio and proof of $100,000/$300,000 auto insurance.
Basic Life Support (BLS) Certification required within orientation period.

Responsibilities

Perform basic patient care activities.
Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.
Assist patients with activities of daily living and transfers/transport.
Report and document abnormal assessment findings to nursing staff.
Keep patient environment clean and orderly.
Maintain complete and up-to-date documentation of patient condition and care.
Escalate changes in patient condition to the physician as appropriate.
