Job summary

Bridge Home Health is seeking a Hospice Aide to join our team serving the Hollister, Gilroy, and Morgan Hill area. The role supports patients under nurse supervision, performing personal care, reporting observations, and ensuring comfort for terminally ill individuals in their homes.

Qualified candidates will hold CNA and HHA certifications in California, be registered with the Home Care Aide Registry, and maintain CPR certification.

Qualifications

• CPR certification current and valid in California.
• Self-directed with minimal supervision and calm demeanor in home settings.

Responsibilities

• Provide personal care (baths, grooming, mobility support) per patient needs.
• Assist with meals, hydration, and feeding as ordered.
• Take vitals as directed and report changes to supervisor.
• Maintain clean, safe patient living area and assist with light laundry.
• Support statements, documentation, and care plans per policy.
