Job summary

New-Age-Hospice is seeking a Certified Nursing Assistant (CNA) or Hospice Aide to provide compassionate in-home patient care in North Houston and surrounding areas. You will support the RN, assist with safe transfers, and document observations to ensure quality care.

Aide must meet state requirements and have reliable transportation. The role emphasizes infection control, respectful communication with patients and families, and adaptability to home settings with flexible scheduling.

Qualifications

• Education aligns with state requirements for nurse aide/ Hospice Aide.
• Minimum one year of hospice/ nursing assistant experience preferred.
• Ability to read, write and report clinical information to care team.
• Reliable transportation and valid driver's license.

Responsibilities

• Provides direct patient care under RN direction and the Aide Plan of Care.
• Assists with safe transfers and ambulation per Hospice policy.
• Reports changes and documents care rendered to ensure continuity.
• Maintains infection control, a clean and safe home environment.
• Communicates respectfully with patient, family and team members.
• Supports efficient operation and time management within the Hospice.
• Participates in the QAPI program and adheres to policies.
• Works in patients' homes with travel to North Houston and surrounding areas.
