Job summary

Qualifications

• At least 1 year experience in hospice or home health care.
• Holds appropriate certificate and/or completed required curriculum/hours; competency evaluated per regulations.

Responsibilities

• Provides direct patient care in the home within defined scope.
• Assists with personal hygiene and daily living activities.
• Records and reports changes in patient condition to RN.
• Maintains infection control and safety policies.
