Adoration Health is seeking a Home Health Social Worker in Warner Robins, GA to provide patient-centered social work support at patients' homes. You will guide families in crisis, assist with accessing benefits, and help develop comprehensive care plans.

The role emphasizes independence with team support and potential advancement within a growing company. Qualified candidates hold a Master’s in Social Work, 5+ years of clinical experience, and an active social work license.

Qualifications

• Master’s Degree in Social Work mandatory.
• Five+ years of clinical social work experience required.
• Licensed Clinical Social Worker or Licensed Medical Social Worker license active and current.
• Ability to perform psycho-social assessments and develop viable care plans.

Responsabilités

• Guide and counsel individuals and families in crisis across diverse situations.
