Job summary

Optimal Care - Evergreen is seeking a Speech Language Pathologist (SLP) to provide in-home speech therapy services in Jackson, MI. The role supports physicians' orders and a treatment plan of care under supervision, with ongoing patient progress reporting and periodic recertifications.

Qualifications include a Master’s or Doctoral degree in Speech Language Pathology and state licensure, plus knowledge of medical terminology.

Responsibilities

• Provides a full range Speech Language Pathology services as ordered by physician
• Participates in patient care planning with other home health personnel
• Takes initial history and performs initial evaluation with a plan of care
• Consults with physicians regarding treatment changes and writes progress reports
• Completes initial evaluation with treatment plan and goals for physician approval
• Performs recertification by physician every 60 days if continued treatment is needed
• Writes progress notes after each visit
• Performs reevaluation and discharge summary as needed
