Job summary

Empath Home Health, a member of Empath Health, seeks a compassionate Home Health RN to deliver individualized care in patients' homes across Labelle, Lehigh Acres, Fort Myers area.

Schedule: M-F 40 hours with weekend/holiday on-call rotations. You will perform OASIS assessments, plan of care, provide skilled nursing, and educate patients and families. Join a mission-driven team dedicated to Full Life Care for All.

Qualifications

Graduate of an approved nursing program with an active Florida RN license.
Prefers BSN or MSN degree.
Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred.
Strong assessment, critical thinking, and patient education skills.
Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

Perform comprehensive patient assessments, including required intervals for OASIS.
Develop, initiate, and revise individualized plans of care as patient needs evolve.
Deliver skilled nursing services and treatments as outlined in the plan of care.
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
Collaborate with the interdisciplinary team to ensure seamless, holistic care.
