Job summary

Humana Inc. in partnership with CenterWell Home Health is seeking a Home Health Registered Nurse to deliver individualized, one-on-one care in patients’ homes.

You’ll work with physicians and clinicians to develop care plans that support recovery and independence. You will administer medications, perform wound care, educate families, and monitor patient status while coordinating with a multidisciplinary team.

Qualifications

• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Med/Surg/ICU/ER/acute experience.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications administration and wound care.
• Develop and implement individualized care plans with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
