Job summary

Prisma Health offers a nursing opportunity in Home Health Midlands with day shift responsibilities. The RN will assess, plan, implement, and evaluate patient care while coordinating with multiple disciplines and agencies involved in the patient’s treatment.

Candidates must hold an active RN license (compact/multistate accepted) and BLS, plus at least one year of RN experience. A valid driver’s license and reliable transportation are required for this role.

Qualifications

• Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
• BLS certification is required.
• A valid driver’s license; an acceptable motor vehicle record; and proof of auto insurance. Driving is required for this position.
• Experience: One (1) year of experience as a Registered Nurse.

Responsibilities

• Assesses, develops, implements and evaluates nursing care for assigned patients.
• Coordinates with other disciplines and agencies involved with patient’s care.
• Documents identified problems, interventions, discharge planning, physician orders and care plan coordination appropriately on admission and throughout certification.
• Participates in chart audits and other related Quality Improvement projects.
