Job summary

Enhabit Home Health & Hospice seeks licensed professionals to provide intermittent skilled care in patients' homes. You will follow physician orders and the established plan of care, while educating patients and families as part of a collaborative healthcare team.

This role requires a valid state license, graduation from an approved clinical education program, and at least one year of licensed professional experience. Strong mobility and technology skills are expected in a patient-care setting.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
