Job summary

Enhabit Home Health & Hospice is seeking a healthcare professional to administer skilled home health care to patients in their homes. You will educate patients and families and collaborate with the care team to follow physician orders and the plan of care.

The role requires graduation from an approved clinical program, licensure in the practicing state, and at least one year of licensed professional experience; CPR certification is preferred and mobility to perform home visits is expected.

Qualifications

• Graduate of approved school of clinical education.
• Licensed to practice in the state where you currently practice.
• At least one year experience as a licensed professional.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
