Job summary

Qualifications
- RN license in good standing.
- Current CPR certification.
- Valid driver’s license and reliable transportation.
- Experience in home health care preferred.
- Must read, write and comprehend English.
Responsibilities
- Perform initial and ongoing assessments of home health clients.
- Regularly re-evaluate nursing needs and outcomes of care.
- Develop and revise the Plan of Care to ensure continuity of care.
- Provide effective communication to clients, families, and team members.
- Apply infection control and universal precautions (OSHA).
- Coordinate services and inform the physician of changes in condition.
- Monitor cases for third party payer compliance.
- Complete timely documentation to ensure policy compliance.
- Supervise and provide clinical direction to LVN/LPN, HHA, CNAs, etc.
- Perform on-call responsibilities and provide on-call services.
