Job summary

ExcelaCare is seeking a qualified registered nurse to administer skilled nursing care in patients' homes, coordinate with a multidisciplinary team, and manage care plans with strong patient communication. The role requires traveling to patient homes and adherence to home health guidelines.

The successful candidate will supervise LVN/LPN and aides, maintain documentation, and participate in ongoing education and quality initiatives within a flexible home health setting.

Qualifications

• Graduate of an accredited school of nursing.
• One year of nursing experience.
• Current State RN license; CPR; current driver’s license and proof of auto liability.

Responsibilities

• Administers skilled nursing care in patients' homes and medications per policy.
• Coordinates care with multidisciplinary team and family; documents progress.
• Performs assessments and develops Plan of Care with physician orders.
• Documents patient status accurately and communicates with care team.
• Supervises LVN/LPN and home health aide, delegating responsibilities.
