Commonheart is seeking a Wisconsin-licensed Registered Nurse for Home Health and Hospice. The role covers assessment, care planning, and ensuring quality nursing services in patients' homes, with on-call duties and travel within a one-hour radius.

The position requires current Wisconsin RN licensure, 2+ years in relevant settings, and emphasizes teamwork, education, and adherence to professional standards. A sign-on bonus is available along with a comprehensive benefits package.

Qualifications

• Must be currently licensed as a Registered Nurse in the State of Wisconsin.
• At least 2 years of experience in medical surgical, home health, hospice or long-term care nursing.
• Certification in Gerontology or Wound Care preferred.

Responsabilités

• Maintains and actively promotes effective communication with all individuals.
• Participate as part of the core team providing care and services and coordinate the plan of care.
• Actively participate in the home health’s quality assurance and performance improvement program.
• Provide education and counseling to the patient and, as needed, to the patient’s family.
• Lead, guide, and direct others in the provision of quality care.
• Practice infection control mitigation at all times.
