The Pennant Group is seeking a licensed Registered Nurse to plan, organize, and direct hospice care in Colorado communities. The RN will lead home-based services, educate patients and families, and work with an interdisciplinary team to deliver compassionate care.

Responsibilities include assessments, care planning, medication administration, and caregiver education, with an emphasis on community health and home-based services. On-call duties and ongoing professional development are expected.

Qualifikationen

• Licensed RN through the State of Colorado.
• Two years of experience in public health, home care, or hospice nursing preferred.
• Demonstrates excellent observation, problem solving, and communication skills.
• Ability to organize and prioritize workload independently.
• CPR certification maintained.

Aufgaben

• Complete initial and ongoing assessments of patient and family to determine hospice needs.
• Develop and revise care plans with patient, family, and interdisciplinary team.
• Administer medications and treatments as prescribed by physician.
• Counsel patient and family on nursing and related needs.
• Provide health instructions and support activities of daily living.
• Coordinate patient care as Case Manager for assigned caseload.
• Attend interdisciplinary group meetings and maintain clinical notes.
