Job summary

CenterWell Home Health seeks a compassionate Registered Nurse to deliver one-on-one, in-home care under the Clinical Manager. You will implement personalized care plans, administer medications, perform wound care, and coordinate with physicians and clinicians to support patient recovery.

You'll educate patients and families, monitor progress, and oversee Home Health Aide documentation while remaining in compliance with driving and safety requirements.

Qualifications

• Current and unrestricted RN licensure.
• CPR certification.
• Experience collaborating with a healthcare team.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes, including meds, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate care records and coordinate with other professionals.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aide work and documentation.

Skills

Med surg ICU ER or acute experience

RN licensure
