Job summary

CenterWell Home Health is seeking a dedicated RN Case Manager to provide admission, case management, and follow‑up skilled nursing visits for home health patients. You will coordinate with physicians and the Clinical Manager to develop, implement, and adjust care plans while ensuring timely documentation and effective discharge planning.

Qualifications

• Diploma, Associate, or Bachelor Degree in Nursing required.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.

Responsibilities

• Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, provide necessary follow‑up as directed by the Clinical Manager.
• Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide hands‑on care, management, and evaluation of the care plan.
• Coordinate appropriate care with Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Report patient care/condition/progress to patient’s physician and Clinical Manager on a continuous basis.
• Implement patient care plan with patient and family to help achieve optimal resolution of needs/problems.
• Coordinate/oversee/supervise Home Health Aides and Personal Care Workers and ensure documentation reflects current plan of care.
• Discharge patients after consultation with physician and Clinical Manager and complete necessary clinical documentation.
• Prepare medical documentation including conferences, contacts, medication changes, re-certifications, progress updates, and care plan changes.
