Job summary

CenterWell Home Health is seeking a compassionate Home Health Registered Nurse to provide one-on-one care in patients' homes under the supervision of the Clinical Manager. You will manage care plans and coordinate with physicians and clinicians to help patients regain independence.

Responsibilities include delivering skilled nursing care, administering medications, wound care, patient assessments, educating families, and guiding Home Health Aides.

Qualifications

• Med surg, ICU, ER, or acute care experience.
• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.
• One year nursing experience (preferred).
• Home Health experience (preferred).

Responsibilities

• Provide skilled nursing care onsite in patients' homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medication, and treatment options.
• Maintain accurate records and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation.
