• Assumes responsibility and accountability for the practice of professional nursing in home health.
• Performs comprehensive assessments of client status including physical, psychosocial, and environmental parameters.
• Completes OASIS data set within 48 hours of referral when applicable.
• Develops and follows an individualized Plan of Care.
• Provides skilled interventions to achieve client outcomes within set timeframes.
• Maintains confidential records and documents in accordance with guidelines.
• Communicates changes to physicians and team promptly.
