Job summary

Adoration Health is seeking a dedicated Home Health RN to join our Gastonia team. Provide 1:1 patient care at home, develop care plans per MD orders, and support families in a compassionate, patient-centered approach.

Position involves Monday–Friday, 8am–5pm, with on-site responsibilities in Gastonia, NC. Requires active RN license, one year of clinical nursing experience, and a valid driver’s license. Join a team emphasizing autonomy with strong team backing.

Qualifications

• RN with current license in state of practice and residence.
• 1 year nursing experience in patient care; home health or geriatrics preferred.
• Valid driver's license, acceptable driving record, and proof of insurance.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligning with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with an interdisciplinary team; maintain timely documentation.
