Job summary

Dartmouth Health Home Care is seeking a Physical Therapist for both full-time and part-time roles serving the Upper Valley area. You will screen, evaluate, and treat patients, coordinating with an interdisciplinary team to deliver high-quality home health care.

Candidates must hold a VT/NH PT license (or compact license) and be a graduate of an accredited PT program; one year of home health experience is a plus. Relocation and comprehensive benefits available.

Qualifications

• Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school in Physical Therapy.

Responsibilities

• Evaluate and treat patients; screen and re-evaluate as part of an interdisciplinary team.
• Develop and implement individualized physical therapy treatment plans.
• Document evaluations, goals, and progress; communicate with care team.
• Teach patients and families the rationale for treatments and procedures.

Education

Licensed Physical Therapist in VT/NH or compact license
