Aufgaben

• Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs and functional level.
• Develops and evaluates the plan of care with patient and caregiver(s).
• Maintains continuing education requirements for the Agency and licensing board.
• Instructs patients and families in transfer techniques and safe use of equipment.
• Provides education to patient and caregiver; coordinates with physicians and other health care practitioners.
• Documents and updates clinical notes and plans of care in a timely manner.
