Job summary

BAYADA Home Health Care, Inspira division, seeks an experienced Physical Therapist (PT) for a full-time in-home visit role serving Cumberland, Gloucester, and Salem Counties in New Jersey. The PT will join a multidisciplinary team delivering rehabilitative care enabling patients to remain safely at home.

The ideal candidate holds a New Jersey PT license, has at least one year of PT experience, and can work independently. Proficiency with EMR and strong communication skills are valued assets.

Qualifications

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from a program approved by the APTA/CAAHEA as indicated by transcripts or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, balance and safety.
• Implement wound care interventions and use evidence-based dressings.
• Develop and implement individualized care plans including manual therapy and gait training.
• Revise care plans and collaborate with nursing on the multidisciplinary team.
• Educate clients and families to promote health, safety and independence.
• Document observations and interventions using a tablet and EMR.
