Job summary

Redmond Assured Home Health is seeking a Physical Therapist to perform assessments and interventions in patients' homes across Washington state. You will determine treatment plans based on evaluations of mobility, endurance, pain, and other medical conditions.

Responsibilities include developing and delivering the therapy plan of care, coordinating with the care team, documenting progress, and ensuring care aligns with state guidelines.

Qualifications

• Current Physical Therapy licensure in the state of Washington.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other health care team members.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
• Functionality in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
