Job summary

HOPE At Home Health Care in Michigan seeks a compassionate Physical Therapist to join our growing team, providing patient-centered therapy in home health settings. You will assess and treat patients according to physician plans and collaborate with families and caregivers.

The ideal candidate has a current state PT license, strong communication skills, and experience in community/home health. Two years of PT experience is preferred.

Qualifications

• Degree from a PT program approved by an accredited organization.
• Licensed to practice as a physical therapist in the state.
• Two years of PT experience; community/home health experience.
• Excellent verbal and written communication and organization skills.
• Current CPR certification.

Responsibilities

• Provides physical therapy services to patients according to a physician's plan of care.
• Assesses muscle strength, mobility, gait, and rehab potential; evaluates home environment.
• Directs physical therapy treatment and instructs patients and caregivers on therapeutic devices.
• Initiates physical therapy program and trains staff and families on therapy goals.
• Prepares progress summaries and participates in discharge planning.
• Supervises physical therapy assistants and coordinates home health referrals as needed.
